Respondents should use the below form to submit their proposal. Each submission package must include a project narrative, a primary offer form, an executed NDA, a signed conflict of interest form, and a redlined ownership or PPA term sheet. Respondents may also include an alternate offer form, audited financial statements, and any additional supporting documents. An automatic email will be sent out to confirm the receipt of the submission package.
